A picture paints a thousand words, so they say. Well in that case I am not going to say too much about this case because the pictures tell us all we need to know. The upper right central incisor was very heavily restored and suffered a root fracture. (This was inevitable given the extent of the post but it did last many years so should be considered a success in it’s own right).
All the usual options for replacement of the tooth were discussed – denture, implant retained crown, conventional porcelain fused to metal bridge work etc but these were all dismissed. The patient want improvements to their smile, overall but did not want any drilling to sound tooth tissue. A laboratory made fiber reinforced composite bridge gives the perfect blend of strength and aesthetics with out the need for drilling to the support teeth. The adjacent teeth that were not involved with the support of the bridge but were part of the smile were enhanced using direct composite fillings, again with out the need for drilling. Whilst this sort of bridge may not be as strong as conventional work it offers the huge advantage of being minimally interventive, repairable and highly aesthetic all at a much lower cost than the porcelains and precious metals that have been used for the past few decades.
